Let us discuss about premature ovarian failure. This is a condition where the ovarian function declines at an earlier age, before the age of 35 years. This can be detected by doing serum FSH and AMH levels on 2 nd day of menses where FSH is high and AMH is low. This could be due to genetic or other causes. An early diagnosis of impending ovarian failure can be done if patients give history of irregular menses or frequent cycles. An ultrasound for antral follicular count should also be done. Treatment of impending ovarian failure is early recourse to IVF. If diagnosed in an unmarried patient who desires future fertility, IVF with oocyte cryopreservation is suggested. If ovarian failure is confirmed, then the only option is IVF using donor eggs
